The test set we send to you contains all documentation and testing swabs for a DNA test. We want to inform you as best as possible and offer to read the documentation here beforehand as a PDF.
You will see that there is not much data you and you neutral witness need to enter.
Each participant needs to give his or her formal written consent to have their DNA tested. For minors all persons having custody need to sign. All participants will than fill in their addresses, as will the neutral witness:
